Reference Letter (3 required for new volunteers)

Please note that all forms should be scanned to a PDF as an attachment (no pictures) and sent via email to volunteerservices@stonybrookmedicine.edu  We do not have access to Google Drive or NETID to open documents.  Be sure to include your name and date of birth in the subject line of the email.  Please do not send in reference letters until your application is approved.  We do not accept walk-in paperwork.
