Reference Letter (3 required for new volunteers)
Please scan and email all forms as a PDF attachment to volunteerservices@stonybrookmedicine.edu
We cannot open files sent through Google Drive or NETID. Include your name and date of birth in the email subject line. Do not submit reference letters until your application has been approved; references must be sent directly by the individuals completing them. Walk‑in paperwork is not accepted.
