Health Plans at Stony Brook University Hospital

AN IMPORTANT MESSAGE FOR PATIENTS OF STONY BROOK MEDICINE:

Thank you for using the world class healthcare offered by the hospitals and physicians of Stony Brook Medicine. Stony Brook Medicine hospitals and physicians participate with many of the companies and plans listed herein; however, some plans are not contracted and are listed as “Out of Network”. Please do NOT assume that a Stony Brook Medicine hospital or physician is “In Network” with your health plan just because the company name is listed. You must look for your specific plan, and then confirm whether the plan is listed as “In Network” or “Out of Network”. We do our best to ensure that the list is accurate and up to date but due to the ever-changing healthcare environment, we cannot guarantee participation. IT IS VERY IMPORTANT THAT YOU CHECK IN ADVANCE WITH YOUR HEALTH INSURANCE COMPANY, HEALTH PLAN ADMINISTRATOR, OR EMPLOYER BENEFITS OFFICE to confirm that it agrees with our list and that the specific Stony Brook Medicine hospital or physician is in network with your plan. We advise you to call the number on the back of your ID card to confirm Stony Brook participation. Accessing an out of network hospital or provider may result in a higher out of pocket cost.

PLEASE NOTE: IF YOU ARE HAVING A MEDICAL EMERGENCY YOU SHOULD IMMEDIATELY DIAL “911” OR GO TO THE NEAREST EMERGENCY DEPARTMENT, REGARDLESS OF YOUR COVERAGE.

Stony Brook University Hospital is a participating provider in many health plan networks. Below is a list of the health plans in which we participate. Some health plans use smaller networks for certain services they offer, so it is important to check whether we participate in your specific plan.
