The Department of Ophthalmology at SUNY Stony Brook participates in a number of managed care panels. When you call for an appointment, you should inquire about your particular plan. While we will attempt to ensure that your visit complies with limits on your coverage, patients should check with their company to be certain. Please bring all necessary insurance information and forms. If your plan requires a referral from you your primary care physician, be sure to to bring written authorization. Please also note that many insurance companies do not cover routine visits unless your plan has a vision care rider. You should check with your insurance company to see if your plan will cover your visit in these cases.

The patient is responsible for payment of all deductibles, co-payments, payment for services not covered by your insurance plan(s) and services that your company deems to be "'not medically necessary".
