2023 Health Care Disparities Action Plans

Effective January 1, 2023, the Joint Commission introduced new organizational requirements to reduce health care disparities, including a new standard in the Leadership (LD) chapter with six new elements of performance (EP). The projects indicated below were identified in accordance with EP 4: “The organization develops a written action plan that describes how it will address at least one of the health care disparities identified in its patient population.”

  • Stony Brook University Hospital:: Colonoscopy wait times and resource utilization in the underinsured gastroenterology patient population
  • Stony Brook Southampton Hospital:: Equitable access for LGBTQ* patients relating to hospital resources and services.
  • Stony Brook Eastern Long Island Hospital:
    • HA1C screening rate in inpatients with a principal or secondary diagnosis code of diabetes
    • Screening for and provision of resources to address food insecurity among inpatients

Additional Organizational Projects

New York State Birth Equity Improvement Project
As part of the New York State Birth Equity Improvement Project, Stony Brook collects and utilizes perinatal data by demographics, including race, ethnicity, gender identity and language. All policies are reviewed to identify potential bias or inconsistencies to promote respectful and standardized care with shared decision-making with Black birthing people. More than 90 percent of nursing staff received education about implicit bias, as well as all medical staff, including Anesthesia. Implicit bias training was implemented for the Admitting Department, and a specific survey was implemented to measure patients experience through a health equity lens.
