Our Mission:

We are a group of Internists and Geriatricians who provide the highest quality Team Based Primary Care for all our Adult Patients.  Through continuous collaboration with patients, caregivers, specialists and community partners we aim to understand each patient’s varied needs with compassion, teamwork, expertise and respect.    

Who We Are:

The Internists and Geriatricians at Stony Brook Primary Care in Setauket and Commack provide primary care for adult patients.  We are a group of faculty physicians as part of SUNY Stony Brook RSOM who not only care for patients but also serve as medical educators and leaders.  As internists we help patients manage their medical problems and provide patients with preventative care and disease prevention to help maintain a healthy lifestyle.  All of our physicians are Board Certified in Internal Medicine by the American Board of Internal Medicine (ABIM)  and many have additional board certification in Geriatrics.

Our Practice:PCMH

Our practices in Setauket and Commack are certified by the National Committee for Quality Assurance (NCQA) as the highest-level Patient Centered Medical Home (PCMH).  This allows us to provide patients with a Team Based Approach to Primary Care. In addition to your assigned Primary Care Physician who will serve as leader of your team, other members may consist of medical fellows, medical residents, medical students, Nurse Practioners, Registered Nurses, Licensed Practical Nurses, Administrative Staff and Social Workers.

As a patient at Stony Brook Primary Care you will have a relationship with a primary care physician who leads your team and takes responsibility for your care.  As the leader of the team your primary care physician will  provide care that is personalized, coordinated, timely and effective.  For routine care like annual visits and follow up visits you will see your assigned primary care physician. At times, for urgent visits your visit may be with another team member.  Anyone who is caring for you will have access to your medical records through our shared Electronic Medical Records system which ensures that your care is timely, safe and communicated directly to your primary care physician

As a patient of our practice, you will have access to all of the top resources of Stony Brook University Hospital, including specialists affiliated with our institution.  Since we work with these specialists every day, we can ensure that your care will be coordinated.  We serve as the anchor for patients who are navigating the complicated visits with many specialists and will ensure coordination of care.

As faculty at SUNY Stony Brook RSOM, the largest teaching hospital on Long Island, we are proud to be part of educating the next generation of health professionals. Our attending physicians supervise our learners who are attending Stony Brook RSOM or graduated from many top ranked medical schools.

Hours of Operation:

In additional to regular daytime hours, we also offer evening, weekend, and early morning appointments for the convenience of our patients as well as same day visits for urgent issues.

Types of Appointments:

-Annual Exam, Annual Medicare Wellness Visits

-Follow up Visits for Routine medical issues

-Urgent Care Appointments, either same day or within a couple of days

-Post Hospitalization/Rehabilitation/ER Appointments, also known as Transition of Care Appointments

-TeleHealth Virtual Visits

- Home Visit Appointments

-Group Visits

Billing and Insurance

We request that any required payments or co-payments are made at the time of the visit. We accept most major insurance carriers and managed health care plans. Please confirm with your insurer that our practice is listed within your network.
