Before Your Visit
To help us serve you better, please bring the following documents and information to your visit:
• Photo ID
• Insurance card and Co Payment
• Referral if required
• A list of current medications, including dosage and frequency
• Any previous imaging reports
• New Patient Packet
If you have not had the chance to fill out the paperwork prior to your appointment, please arrive 30 minutes before your scheduled appointment.
If you have completed your paperwork, please arrive 15 minutes before your scheduled appointment to allow for registration and check in.
