Our Services
Medical Nutrition Therapy is a specialized, evidenced-based approach to managing chronic conditions through personalized nutrition care. For people with diabetes, MNT focuses on making strategic dietary modifications that support stable blood sugar levels, improve overall health, and prevent complications. Medical Nutrition Therapy goes beyond basic diet advice. It involves working 1:1 with a Registered Dietitian Nutritionist (RDN) to develop a customized eating plan that aligns with individual health needs, lifestyle, and goals. MNT typically includes:
- Initial Comprehensive Assessment
- Personalized Nutrition Plan
- Goal Setting
- Ongoing Support & Adjustments
Diabetes Self-Management Training (DSMT)
Diabetes Self-Management Training (DSMT) is a comprehensive approach that empowers people to manage their diabetes more effectively through education, practical skills, and personalized support. By understanding how diabetes affects the body and learning how to make daily choices that improve one’s health, DSMT participants are better equipped to achieve optimal blood sugar control, reduce their risk of complications, and improve their overall quality of life. DSMT provides a comprehensive framework that empowers patients to navigate the various aspects of diabetes management with confidence. Through DSMT, patients learn to:
- Monitor Blood Sugar
- Use Medications Effectively
- Plan Balanced Meals
- Build a Sustainable Physical Activity Program
- Problem-Solve for Daily Challenges
Our DSMT program is designed to meet individual's needs whether it is providing tailored guidance through 1:1 sessions or offering group education classes where patients can obtain peer support and shared learning experience.
Our DSMT classes are provided bimonthly and we offer both morning and evening sessions.
Topics include:
- Overview of Diabetes (Type 1 & Type 2)
- Diabetes Medications
- Decreasing Risk for Acute & Chronic Complications
- Nutrition and Meal Planning
- Blood Sugar Monitoring and Interpretation (glucometers, CGMS)
- Psychosocial adjustment and Stress Management
- Health Maintenance (eye exams, foot exams, etc.)
A referral from your treating physician is required to schedule an appointment for MNT or DSMT.
Many commercial insurance plans cover Medical Nutrition Therapy (MNT) and Diabetes Self-Management (DSMT), depending on the plan your employer negotiated with the carrier. To determine if your insurance with cover these services, we highly recommend you contact your insurance company before arriving to your appointment. Please ask your insurance representative how many “hours” or “units” per year you are entitled to for each service. Please inform the representative that the services are billed as outpatient hospital services. Please ask also if you are covered for Telehealth visits.
