To help provide the best care to your patients, Stony Brook University Hospital offers physicians access to their patients’ electronic medical records. Community physicians and their designated employees can apply for access simple by filling out an application and signing a confidentiality agreement.
Applications can be obtained by contacting the Physician Outreach Office at (631) 638-4040 or if you are a Community Physician or staff member with remote access to the Community Physician Portal go to: www.stonybrookmedicine.edu/CommunityPhysicianPortal
