Request an Appointment

Appointments are available Monday through Friday. Evening appointments are available on select days. The office is closed on weekends and major holidays.

If you are an established patient and would like to schedule, cancel or change an appointment, please call (631) 689-5390. For patients that want to leave a message for their provider, including cancellations, the office has a 24-hour voicemail.

To Become a New Patient

If you are a new patient 18 years and older, you must complete the New Patient Request Form and Patient History Form located below in order to start the new patient process. Forms can be filled out as PDFs on a device, or they can be printed and handwritten. Please email or fax the completed forms with a copy of the front and back of your insurance card (for Medicaid patients, we will need the Medicaid ID # as well) to either aibh@stonybrookmedicine.edu, or fax (631) 689-5395. An administrative staff member will contact you within three to five business days regarding your new patient appointment request.

New Patients Pending an Appointment

If you were approved as a new patient, please complete the New Patient Packet and Patient Registration Packet. Paperwork must be completed prior to booking an appointment. Please email or fax a copy of the completed forms with a copy of your driver's license. Upon receipt of your packet and identification, an administrative staff member will contact you to schedule an appointment. Please arrive 15 minutes prior to your scheduled appointment and be sure to bring a valid photo ID and your insurance card. Good Faith Estimate

If you are uninsured, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your healthcare provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.

Telehealth Services for Patients

At All Island Behavioral Health, your health and safety are our top priorities. We have added a Telehealth service to our practice so if necessary we can continue to serve your medical needs virtually.
